Relationships
The
Center works to increase community capacity throughout Chicago by working
in partnership with neighborhood organizations and coalitions. The Center
helps communities to access and analyze health and health systems data,
shares information about grants and other opportunities, and facilitates
linkages to other coalitions, community organizations, and City agencies.
The Center supports community coalitions
comprised of community residents, health and social service providers,
the business and faith communities, and other key stakeholders, and
works to develop and then implement community health improvement plans.
This approach is unique in that it is a community-led process within
a structured framework, data collection is driven by the community,
technical assistance is provided by a large public health agency, and
appropriate citywide and community-level roles for health improvement
are identified.
