Chicago
has 77 formally-designated community areas; Community Health Profiles,
which are available for each, contain a mix of health status and health
resource data. Each profile presents information for the specific community,
and in some instances, comparable information for the city as a whole.
In addition to a map that depicts the locations of health care facilities,
each community profile includes:
